We are currently accepting new patients! If you would like to establish with our practice, please send in your paperwork and we will call you to schedule an appointment.

During the off-season, we are typically able to schedule patients within one to two days. BUT...please be advised, there is usually a longer wait time during season. 

Once established, your wait time will be much shorter.

For urgent matters, please go to an Urgent Care facility in the meantime.

BELOW ARE OUR NEW PATIENT REGISTRATION FORMS.

IMPORTANT:

Dr. Williamitis is NOT a Workmen's Comp. physician but does see patients who have been injured in Motor Vehicle Accidents.

We will need the new patient paperwork emailed or faxed to us before we can schedule your first appointment. You may also drop the packet off in person if it is more convenient for you. Your first visit will consist of establishing care with one of our Nurse Practitioners or with Dr. Williamitis as his schedule allows. In this visit you will discuss any questions and concerns you may have, medication refills, etc. We do not schedule yearly physicals for your first appointment, but you may schedule one in the future. Bring your insurance card and driver's license or ID to your first visit where it will be scanned into your chart.

Please arrive 5-10 minutes before scheduled appointment time. In our effort to keep wait times as short as possible, we do not accept arrivals greater than 10 minutes late when we have a full schedule, 

which is most days, and you will be rescheduled.

PLEASE NOTE: When submitting through email, attach each family member's new patient paperwork as a scanned PDF document attachment, not separate pages .
