BCBS: My Blue, Select Bronze, Blue Select, Florida Blue Medicare

            Advantage HMO.

CIGNA: EPO, Local Plus, Priority Health.

HUMANA: Gold HMO.

MEDICAID: This includes Aetna Healthy Kids (Better Health), Sunshine,

                    Ambetter, Wellcare, Molina.

OTHER: Emblem, Gerber, GHI, Prestige Health, Staywell, Wellmed, 

               Detego Health.
